    • Ensure compliance to Juniper Village’ philosophy and hallmarks as well as all written policies and procedures that govern the operation of the Health and Wellbeing Department.
    • Embraces Culture Change and resident-centered philosophies
    • Ensure compliance with documentation and record keeping to meet current regulatory compliance standards.
    • Continually assess residents to identify any change in condition, mood, behavior, or level of function to insure appropriate interventions to maintain the residents’ highest practicable physical, mental and psychosocial wellbeing.
    • Assist in the coordination of effective communication among Wellness Associates, residents, and families.
    • Maintain liaison with residents and family members.
    • Through ongoing supervision, oversight, and delegation ensures the provision of a continuous high quality of service within the Neighborhood.
    • Assist in establishing rapport among Wellness team members to facilitate teamwork.
    • Assist in the recruitment and selection of competent Wellness Associates: CNAs.
    • Perform continuous, ongoing supervision of all Neighborhood Wellness team members, including but not limited to training, disciplining, counseling, motivating, and evaluating.
    • Ensure all Wellness staff complete mandatory In Touch Orientation and in service training.
    • Ensure that all staff follows established safety regulations, including fire protection, infection control, bloodborne pathogens, hazardous waste and quality assurance procedures.
    • Carry out QAPI roles and responsibilities as assigned.
    • Follow established policies and procedures in support of QAPI efforts.
    • Make written and oral reports to the Director of Wellness concerning the operation of the Neighborhood
    • Work in concert with other members of the Health and Wellbeing Leadership Team.
    • Assist in the coordination of health care services with other resident services to ensure continuity of care.
    • Responsible for verifying all medical forms completion prior to resident move-in within the neighborhood
    • Continually assess residents to identify and insure appropriate care and service.
    • Coordinate and implement regularly scheduled neighborhood meetings
    • Complete all administrative paperwork as required.
    • Administer Mantoux and Hepatitis B Vaccinations.
    • Organize annual Influenza Vaccinations.
    • Assist with medication administration.
    • Work in conjunction with resident’s attending Physician to make certain highest quality of service is being provided.
    • Submit recommendations to the Director of Wellness for budget completion, equipment, and supplies.
    • Ensure adequate staffing pattern of Resident Associates: CNAs within authorized hour guidelines at all times.
    • Develop work schedules and assignments to ensure staff accountability and task completion.
    • Ensure adequate supplies and equipment is available to meet the day to day operational needs of the Wellness Department.
    • Maintain confidentiality of all resident and staff information.
    • Assists in the review all complaints, grievances, and incidents and assists the Director of Wellness in the development of a written plan of correction.
    • Serve as networking team member, as directed.


    • LPN or RN license required. A current LPN or RN license in good standing.
    • Previous management experience in healthcare or service related setting.
    • Knowledge of all applicable governmental regulations AND MDS.
    • Knowledge of the physiology and psychology of the older adult.
    • Must be able to read, write, speak, and understand the English language. Must be computer literate.
    • Must not pose a direct threat to the health or safety of other individuals within the community.
    • Attends and participates in mandated yearly training programs as well as all required meetings.
    • Must be able to manage time efficiently so as to achieve goals.
    • Adheres to all Human Resources policies, procedures, and code of conduct.
    • Respects confidentiality of information and Resident’s rights.
    • Perform job in a safe manner, using proper body mechanics, infection control, etc.
    • Responsible for proper care of all property, equipment, supplies and financial assets involved in the performance of the job.
    • Perform other duties as required.
